Cigna's plans will cover rehabilitation inpatient treatment, but there are limitations. In most cases, Cigna will only cover a portion of the total cost of your rehab facility. If you have a deductible on your policy, it will have to be met before any payments are made by Cigna. Once that happens, they may pay up to 100% of approved charges for an inpatient stay or 20% after that point until the end of your stay.

Cigna's outpatient coverage is more straightforward: as long as you have met your deductible and co-payments (which could range between 20% and 50%), they will reimburse up to 80% of approved charges at any given time during treatment.

The Coverage Levels & Plans for Cigna

The levels are: bronze, silver, gold, and platinum. There is also a basic benefit plan that does not allow for rehab treatment but does cover some other services related to treatment, like therapy visits or support groups. If you want to go this route, it is only available on their Silver or Gold plans in most states though there are some exceptions where it can be purchased even if you are on one of their higher plans as well, such as Florida or Arizona, where they do offer Basic at all levels due to state regulations requiring insurers provide access regardless what type of policyholder they may be (employee vs self-employed).
